In consideration of being accepted by First Baptist Church Hildebran for the participation in ANY AND ALL EVENTS, TRIPS, AND BUS TRANSPORTATION from JANUARY 1 THROUGH DECEMBER 31, 2025, I/we do hereby release, forever discharge and agree to hold harmless First Baptist Church Hildebran and its directors, host households, drivers, etc., thereof from any and all liability, claims or demands for personal injury, sickness or death, as well as property damage and expenses, of any nature whatsoever which may be incurred by the undersigned and the youth that occur while said youth is participating in the above described trip/activity.
Furthermore, I/we the parent(s)/guardian(s) of this youth participant, hereby grant our permission for him/her to participate fully in said trip/activity/transportation, and hereby give permission to have said youth participant taken to a doctor or hospital and hereby authorize medical treatment and assume the responsibility of all medical bills, if any. Should it be necessary for the youth participant to return home due to medical reasons, disciplinary action or otherwise, I/we hereby assume all transportation costs.
